Documentation Guidelines for Hearing Loss:

It is helpful if students who are deaf or hard-of-hearing provide AccessibleNU-Q with a current audiogram by a licensed audiologist. Additionally, current medical documentation helps make the most accurate accommodation determinations.

An audiogram showing evidence of a hearing loss will help determine services and priority registration. Including information about the functional limitations caused by the hearing loss in the report is helpful if the student is seeking additional academic accommodations, which may include (although this list is not exhaustive):
