Health Insurance Requirement and Hamad Health Card

NU-Q requires all currently enrolled students to submit front and back copies of their valid State of Qatar national health insurance card. Submissions are accepted by NU-Q Counseling, Health, and Wellness and are due by these dates.

Students who do not currently reside in Qatar

Health Card Application

If you are preparing to attend NU-Q but currently live outside of Qatar and do not possess a Qatar identification card, complete the health card application form

Special Instructions

  • Fill in “Mother’s Name” with your mother’s full name (first, middle, last) (this is in upper right corner of form)
  • Leave blank the “Electricity #”
  • Leave blank numbers 1-3
  • Complete numbers 4-9 with your information
  • Leave blank numbers 10-17
  • Sign your name by “Applicant’s Signature”

How to Submit the Form

Students starting at NU-Q in fall semester should submit their documents electronically in the portal.

Students starting at NU-Q in any other semester should submit their documents in person or send them via email to patricia.collins@northwestern.edu by these deadlines. Student Affairs will assist students in obtaining the Qatar national health insurance card once documentation is received and a resident permit is obtained for the student.

Students who currently reside in Qatar

Health Card

If you are preparing to attend NU-Q and you possess a Qatar identification card, you are responsible for obtaining and renewing your own State of Qatar national health insurance card.

If you do not have a national health insurance card:

You can apply for it by completing the health card application and submit it to your nearest primary health care center.

If you already have a national health insurance card:

Check the validity and renew your card, if necessary, online via Hukoomi - Health Card eService.

Health insurance requirement

NU-Q requires all currently enrolled students to participate in a private comprehensive health insurance plan. The policy, at no cost to the student includes access to medical, financial, and travel benefits. No additional documents are required for your enrollment. Please note: there is no guarantee NU-Q will provide this free service in the future. 

Beginning August 1 of each enrollment year, sophomores/juniors/seniors may retrieve their new insurance card from department of student affairs reception. Replacement costs for lost cards are incurred by the student. Freshmen will receive their insurance cards during freshmen orientation and can retrieve them after orientation from department of student affairs reception.

Currently enrolled and incoming freshmen will be automatically registered for the policy, which is valid yearly from August 1 through July 31. Beginning August 1, 2018, students not currently enrolled (exception information can be found here) are no longer covered by the university provided health insurance.
