Health Insurance Requirement and Hamad Health Card

NU-Q requires all full-time enrolled students to submit front and back copies of their valid State of Qatar national health card by the due dates listed here.

Instructions:

International Students (sponsored by Qatar Foundation)

Within two weeks of receiving your Qatar Identification Card, please submit the health card application, a housing verification letter from QF housing, front/back copies of your Qatar Identification card, and a hard copy of a passport size photo.  

Step 1:

Special Instructions For the Hamad Card application

  • Fill in “Mother’s Name” with your mother’s full name (first, middle, last) (this is in upper right corner of form) and write your Qatar local cell phone number above your "mother's name"
  • Leave blank the “Electricity #”
  • Leave blank numbers 1-3
  • Complete numbers 4-9 with your information
  • Leave blank numbers 10-14
  • If you have been assigned to the womens residence hall, complete number 15 with "Qatar Foundation, Building 39, Zone 52, Street 2731"
  • If you have been assigned to the mens residence hall, complete number 15 with "Qatar Foundation, Building 22, Zone 52, Street 2710" 
  • Complete number 16 with your Qatar Identification number found on your Qatar Identification Card

Step 2:

Request a letter from housing verifying your residence in QF housing

Upload your Hamad application, housing verification letter, and front and back copies of your Qatar Identification card here

Step 4:

Obtain a passport size photo of yourself and submit it to Counseling and Wellness.  Digital photos are not accepted.   

QF sponsored students are responsible for checking the validity and requesting its renewal by emailing wellness@qatar.northwestern.edu.  

Domestic Students

Health Card

If you are preparing to attend NU-Q and you are not sponsored by Qatar Foundation, you are responsible for obtaining and renewing your own State of Qatar national health card.  Once you have your Hamad health card, upload front and back copies of the card here.

If you do not have a national health card:

You can apply for it by completing the health card application and submit it to your nearest primary health care center.

If you already have a national health insurance card:

Check the validity and renew your card, if necessary, online via Hukoomi - Health Card eService.

Health insurance requirement

NU-Q requires all full time enrolled students and part time enrolled seniors in their last semester to participate in a private comprehensive health insurance plan. The policy, at no cost to students, includes access to medical, financial, and travel benefits. No additional documents are required for your enrollment. Please note there is no guarantee NU-Q will pay for this requirement in the future.   

Full time enrolled students, part time enrolled seniors in their last semester and incoming students will be automatically registered for the policy, which is valid yearly from August 1 through July 31. Students (not including new incoming students) who register for fall classes after July 1 will be added to the policy after the university's official census date.  

Students who do not register for the fall semester but do register for the spring semester should contact Counseling and Wellness to initiate the enrollment process.  Please allow for 7 business days for health insurance to be activated.  

Beginning August 1, 2018, students not currently enrolled (with the exception of students on their two semesters of an approved MLOA) are no longer covered by the university provided health insurance. 

Beginning August 1 of each enrollment year, sophomores/juniors/seniors may receive their insurance card electronically via their NU-Q email address and may retrieve the hard copy from Counseling and Wellness.  Incoming/first year students may receive their insurance card electronically via their personal email and the hard copy will be distributed to them during new student orientation.  Replacement costs for lost cards are incurred by the student.

Exception for the 2022-2023 academic year 
Incoming first year students will receive health insurance beginning July 1-31, 2022 under the 2021-2022 policy. They will not receive hard copies of the health insurance card but will have access to electronic health insurance cards.  On August 1, 2022 incoming first year students will be enrolled in the 2022-2023 policy.  They will receive hard copies of the health insurance card as well as access to electronic health insurance cards.  

Effective immediately March 3 2022: Al Jazeera Medical Center and all its branches ( Al Wakra, Abosedra, Muaither, Doha) are no longer part of Seib's direct billing network and any claims submitted for reimbursement will not be accepted.
